What is Employer Paid Services?

Employer Paid Services or EPS consist of several medical services that Employers require all their employees to go through before starting any regular day-to-day job tasks.

Employer Paid Services (EPS) that we offer here in our San Clemente office are:

  • Department of Transportation (DOT) Physical for Commercial Driver License (CDL) – New application or Renewal (click here)
  • NON-DOT Work Physical
  • Pre-Employment Work Physical
  • Fit for Duty Physical – depending on specific job descriptions
  • Medical Release Physical
  • Drug Screening (specific number of panel or drugs tested can be accommodated upon exclusive contract)
  • Tuberculosis Skin Test (TB Test or PPD)
  • Electrodiagram or EKG

If prospective employers have specific or unique requirements that you want to discuss with our facility, please feel free to give us a call and we will do our best to accommodate your request.

Most Employer Paid Services may be billed directly to the contracted Employers or be charged at Point of Service. Any prospective employers who prefer to tour our facility before making any decision, please contact us to set up an appointment. Our Practice Administrator and Chief Medical Officer will be available to meet with you in person.

All Employer Paid Services may be paid directly by Employees at the time of service. An itemized invoice will be created at the end of the service for reimbursement by the employers.

Below is the link to a Registration and Authorization form for any prospective employers. Please email or fax the completed form to us before the initial service is performed. The employee may also bring the completed form at the date of service.
